Important Notice - City of Belleville Proof of Vaccination Requirement as of September 22, 2021
We previously communicated the Province of Ontario's proof of identification and full vaccination requirements for certain businesses and settings that included the Quinte Sports and Wellness Centre. This requirement took effect on September 22, 2021.

The health and safety of City employees and individuals entering City buildings is a priority for the City of Belleville. To support this, the City has introduced a proof of identification and COVID-19 vaccination policy to ensure individuals entering City buildings are fully vaccinated against COVID-19, with some exceptions.
Starting September 22, 2021, the City of Belleville will require all individuals 12 years of age and older wishing to enter City buildings to provide proof of identification (showing name and date of birth) AND proof of being fully vaccinated against COVID-19 each time they seek entry to a City building, including the Quinte Sports & Wellness Centre.

Individuals who do not provide proof of identification AND proof of being fully vaccinated against COVID-19 will be directed to access City services, where available, online or by phone. Clients will not be able to access the Quinte Sports & Wellness Centre without showing full proof of vaccination and identification (photo ID is not required for individuals under 18).
In certain limited circumstances, as noted below, exemptions from these requirements may apply. Acceptable exemptions to the Policy are:
  • Where an individual is not able to obtain a COVID-19 vaccine for a medical reason AND the individual provides written proof from a physician, which is satisfactory to the City, that a medical exemption is required.
  • On or before November 12, 2021, where the individual is a youth under 18 who is actively participating in an organized sport (sports leagues, organized pick-up sports, dance classes and swimming classes), and coaches and officials. This exemption applies ONLY to individuals seeking entrance to the Quinte Sports & Wellness Centre. This exemption will not be available after November 12, 2021.
  • On or before November 12, 2021, where the individual is a client attending a physiotherapy or massage appointment. This exemption applies ONLY to individuals seeking entrance to the Quinte Sports & Wellness Centre for the period of time they will be attending their physiotherapy or massage appointment – once the appointment is completed the individual is required to exit the building. This exemption will not be available after November 12, 2021.
  • Where the individual is a City employee and they are seeking entry to the City building in their capacity as a City employee. City employees are covered by the Mandatory Covid-19 Vaccination Policy.
September 22 to November 12, 2021
Vaccination Proof Required
  • 12 years of age and older, unless exempted
Vaccination Proof Not Required
  • Medical exemption (with documented proof from a physician)
  • Youth under 18 who is actively participating in an organized sport at the QSWC (sports leagues, organized pick-up sports, dance classes and swimming classes), and coaches and officials (exemption not available after November 12, 2021).
  • Clients attending a physiotherapy or massage appointment at the QSWC (exemption not available after November 12, 2021).
  • City employees seeking entry to a City building in their capacity as a City employee (employees are covered by the Mandatory COVID-19 Vaccination Policy)

To ensure a safe and positive experience, everyone is to be treated with respect, kindness and patience as we implement this Policy. Individuals who do not conduct themselves in this manner will be asked to leave City buildings and entrance areas immediately. 
The Recreation, Culture & Community Services Department extends our sincere gratitude for your ongoing patience as we implement this policy at our facility. The line-ups are expected to be a bit longer as we confirm proof of vaccination upon entering the QSWC in addition to screening and contact tracing.

Proof of Identification and Full Vaccination Definition

Full Vaccination 
An individual is considered fully vaccinated if they have received:
  • the full series of a COVID-19 vaccine authorized by Health Canada, or any combination of such vaccines,
  • one or two doses of a COVID-19 vaccine not authorized by Health Canada, followed by one dose of a COVID-19 mRNA vaccine authorized by Health Canada, or
  • three doses of a COVID-19 vaccine not authorized by Health Canada; and
  • they received their final dose of the COVID-19 vaccine at least 14 days before providing the proof of being fully vaccinated.
Health Canada approved vaccines:
  • 2 doses: Pfizer-BioNtech, Moderna, AstraZeneca/COVISHIELD
  • 1 dose: Janssen/Johnson & Johnson
  • mRNA vaccine: Pfizer-BioNtech, Moderna
Identification Proof
Validation of vaccination will be based on two key identifiers:
1. Name of the identification holder; and
2. Date of birth
A photo identification is not required for individuals under age 18. 
Proof of identity can be established using documentation issued by an institution or public body, provided it includes the name of the holder and date of birth. 
Examples of identification documents that may be used to confirm the identity of the holder of the vaccine receipt include:
• Birth certificate
• Citizenship card
• Driver’s licence
• Government (Ontario or other) issued identification card, including health card
• Indian Status Card/Indigenous Membership Card
• Passport
• Permanent Resident card 
Validation of identification is considered to have been completed/successful when the name and date of birth of the presenter of the vaccination receipt and the name and date of birth on the identification document match. If the name and date of birth on both documents do not match, the individual will not be allowed to enter the business or organization.
